Washington County Hospital and Medical Group would like to inform residents of their new Diabetic Education outpatient clinic.  The program has been developed to follow the American Diabetes Association’s guidelines for Diabetes Self-Management Education Programs and will adhere to their requirements.  This DSME program will cycle through nine curriculum content areas related to diabetes and patient self-management.  At this time it will be a monthly education class to be held on the fourth Wednesday of every month.  Each patient is required to have a referral from their medical provider and a clinical diagnosis of diabetes.  Appropriate referrals include patients with recurrence of elevated glucose levels, recurrent hypoglycemia, a change in their treatment, a new diagnosis, and those at high risk related to complications and/or co-morbidities.  Each patient will have an evaluation on his or her individual needs and a plan of care will be developed accordingly.  Again, the monthly education will begin on January 27th, in the Board Room of the hospital at 2 PM.  Contact your medical provider to complete the referral form.  If you have any question call 618-327-2225 or visit the WCH website at www.washingtoncountyhospital.org.
